PT Lotte Chemical Titan Indonesia didirikan dengan nama PT Indofatra Plastik Industri yang dicatat dalam Akta Notaris Ny. Rukmasanti Hardjasatya, S.H. Nomor 19 Tanggal 09 Desember 1987 yang telah disahkan oleh Menteri Kehakiman Republik Indonesia melalui Surat Keputusan Nomor C2-6603.HT.01.01.Th.88 Tanggal 30 Juli 1988. LCTTBK memulai kegiatan usaha komersial di bidang industri plastik dengan lini produksi pertama dengan kapasitas terpasang awal 8.000 metrik ton per tahun.
Kemudian pada tanggal 18 Juli 1988 merubah nama menjadi PT Fatrapolindo Nusa Industri dan merubah bentuk usaha menjadi perusahaan terbuka setelah memperoleh keputusan dari Menteri Kehakiman dan Hak Asasi Manusia dalam Surat Keputusan Nomor C-5565.HT.01.04-TH.2001 Tanggal 23 April 2001 dan mencatatkan sahamnya pada Bursa Efek Indonesia dengan kode saham FPNI.

Shift Paramedic
Job Responsibilities:
Assesses, treats, evaluates, and follows up on occupational injuries and illnesses incurred by sick or injured employees
Assesses, treats, evaluates, refers, and follows up on all non-occupational injuries and illnesses. Interfaces with employees and the company doctor
Support company doctor on treatment of work-related injuries and illnesses with ensuring compliance with national and industrial regulations
Support the plan and implementation of company medical screening program including pre-employment, annual, and special medical examination
Maintaining accurate records of patient diagnoses, test results, treatments, and follow-up care needs
Performing wound care, such as cleaning and bandaging them, as well as drawing blood, urine samples, and other body fluids for lab work
Administering medications and treatments, then monitoring patients for side effects and reactions
Perform routine inspection related to health and hygiene including food hygiene, medical inventory tools and equipment, first aid box, stock of medicine, and other necessary inspection to be determined later
Administers and documents company occupational health program which including but not limited to: blood borne pathogen, drug and alcohol test, pandemic preparedness, and health promotion
Collaborate with other departments within the organization to promote a culture of health and wellness
Perform administrative duties as needed, such as preparing reports and participating in meetings
Maintains employee medical records and documentation including accident reports (on medical side), medic monthly reports, and other health-related statistics
Support the purchase and order of medical supplies including clinic tools and consumables, as well as medicine stock
Involve as medical team member during company emergency or crisis situation
Job Requirements:
Min. Diploma 3 in Nursing and has a nurse’s registration certificate (STR) with 3 – 8 years experiences
Certified Hiperkes (Regulation no.1/1979) certification
Certified BTCLS (Basic Trauma Cardiac Life Support)
Retain the valid license (STR) and professional certifications as an Occupational Nurse
Proficient knowledge of relevant nursing laws, policies, and procedures
Excellent communication skills, both verbal and written
Proficient computer skills
Experienced & capable as Health Subject Trainer
Preferred has a driving License Class A
Willing to be work in shift
Willing to be placed in Merak Plant (Cilegon)
